On a constat\u00e9 que cinq isolats en Ontario \u00e9taient g\u00e9n\u00e9tiquement li\u00e9s par s\u00e9quen\u00e7age du g\u00e9nome entier (SGE). L\u2019Ontario a commenc\u00e9 \u00e0 enqu\u00eater sur ces cas.<\/li>\n<li>Par la suite, le PNSME a signal\u00e9 une augmentation des cas d\u2019infections \u00e0 <em>Salmonella<\/em> Newport le 15 mai\u00a02020 \u00e0 l\u2019\u00e9chelle nationale, ainsi qu\u2019en Colombie-Britannique, en Alberta et en Ontario.<\/li>\n<li>Le 21 mai\u00a02020, le Laboratoire national de microbiologie (LNM) a affich\u00e9 une grappe SGE de neuf cas d\u2019infections \u00e0 <em>Salmonella<\/em> Newport (ON\u00a0=\u00a06, AB\u00a0=\u00a01, CB\u00a0=\u00a02) sur le babillard h\u00e9berg\u00e9 sur la plateforme du R\u00e9seau canadien de renseignements sur la sant\u00e9 publique (RCRSP), sous le code de grappe\u00a02005NEWWGS-1ON-MP de PulseNet Canada (PNC).<\/li>\n<li>Une t\u00e9l\u00e9conf\u00e9rence d\u2019\u00e9valuation du comit\u00e9 de coordination de l\u2019enqu\u00eate sur l\u2019\u00e9closion (CCEE) a eu lieu le 25\u00a0mai\u00a02020, et les partenaires ont convenu de former un CCEE national.<\/li>\n<\/ul>\n<p><strong><u>Enqu\u00eate \u00e9pid\u00e9miologique<\/u><\/strong><\/p>\n<ul>\n<li>L\u2019information sur l\u2019exposition a \u00e9t\u00e9 re\u00e7ue pour 15 des 18 cas (questionnaire g\u00e9n\u00e9rateur d\u2019hypoth\u00e8ses\u00a0=\u00a08, questionnaire cibl\u00e9\u00a0=\u00a07, questionnaire provincial\u00a0ordinaire\u00a0=\u00a011).\n<ul>\n<li>7 cas ont \u00e9t\u00e9 r\u00e9interrog\u00e9s par la DGE de l\u2019ASPC au moyen d\u2019un questionnaire cibl\u00e9 (ON\u00a0=\u00a02, AB\u00a0=\u00a01, CB\u00a0=\u00a04)<\/li>\n<li>3 cas sur 18 ont \u00e9t\u00e9 perdus pendant le suivi (ON\u00a0=\u00a01, AB\u00a0=\u00a01, QC\u00a0=\u00a01)<\/li>\n<\/ul>\n<\/li>\n<li>Les aliments suivants ont \u00e9t\u00e9 signal\u00e9s \u00e0 des fr\u00e9quences plus \u00e9lev\u00e9es que pr\u00e9vu lors des nouvelles entrevues (voir <strong>l\u2019annexe B<\/strong>), mais aucun point commun n\u2019a \u00e9t\u00e9 observ\u00e9 dans les renseignements sur la marque, les lieux d\u2019achat ou les fournisseurs\u00a0:\n<ul>\n<li>6 cas sur 12 ont d\u00e9clar\u00e9 avoir consomm\u00e9 des graines de lin<\/li>\n<li>6 cas sur 12 ont d\u00e9clar\u00e9 avoir consomm\u00e9 des graines de s\u00e9same<\/li>\n<li>6 cas sur 14 ont d\u00e9clar\u00e9 avoir consomm\u00e9 des bleuets<\/li>\n<li>7 cas sur 12 ont d\u00e9clar\u00e9 avoir consomm\u00e9 des amandes<\/li>\n<li>5 cas sur 12 ont d\u00e9clar\u00e9 avoir consomm\u00e9 des noix de Grenoble<\/li>\n<\/ul>\n<\/li>\n<li>Graines de chia\n<ul>\n<li>12 cas sur 14 ont d\u00e9clar\u00e9 avoir consomm\u00e9 des graines de chia<\/li>\n<li>3 cas ont d\u00e9clar\u00e9 avoir achet\u00e9 des graines de chia de la marque \u00ab\u00a0Smile\u00a0\u00bb\n<ul>\n<li>1 cas les a achet\u00e9es au magasin A de la cha\u00eene Healthy Lifestyle Emporium<\/li>\n<\/ul>\n<\/li>\n<li>1 cas a d\u00e9clar\u00e9 avoir achet\u00e9 des graines de chia de la marque \u00ab\u00a0Nature\u2019s Planet\u00a0\u00bb<\/li>\n<\/ul>\n<\/li>\n<li>8 cas n\u2019avaient pas d\u2019information sur la marque\n<ul>\n<li>1 cas les a achet\u00e9es au magasin B de la cha\u00eene Healthy Lifestyle Emporium<\/li>\n<li>1 cas les a achet\u00e9es dans une \u00e9picerie ind\u00e9pendante locale<\/li>\n<li>Les relev\u00e9s d\u2019achat ont confirm\u00e9 que 2 cas avaient achet\u00e9 des graines de chia de la marque \u00ab\u00a0Smile\u00a0\u00bb<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><strong><u>Enqu\u00eate de laboratoire<\/u><\/strong><\/p>\n<ul>\n<li>Tous les cas confirm\u00e9s ont \u00e9t\u00e9 consid\u00e9r\u00e9s comme \u00e9tant apparent\u00e9s avec 0 \u00e0 6 diff\u00e9rences d\u2019all\u00e8les selon l\u2019analyse wgMLST (voir l\u2019annexe C).<\/li>\n<li>La grappe \u00e9pid\u00e9mique se situait \u00e0 moins de 25 all\u00e8les pr\u00e8s d\u2019une grappe ant\u00e9rieure de quatre cas ontariens de 2019 (1908NEWWGS-1ON). Les dates d\u2019apparition des sympt\u00f4mes des cas appartenant \u00e0 cette grappe allaient du 8 ao\u00fbt\u00a02019 au 2 septembre\u00a02019. La source de cette \u00e9closion n\u2019a jamais \u00e9t\u00e9 confirm\u00e9e, bien qu\u2019on ait soup\u00e7onn\u00e9 un type de produit de noix ou de graines.<\/li>\n<li>Il n\u2019y a aucun isolat non clinique \u00e0 moins de 25 all\u00e8les pr\u00e8s de la grappe de l\u2019\u00e9closion dans la base de donn\u00e9es PulseNet Canada et aucune correspondance avec la grappe de l\u2019\u00e9closion n\u2019a \u00e9t\u00e9 identifi\u00e9e aux \u00c9tat\u2011Unis.<\/li>\n<\/ul>\n<p><strong><u>Enqu\u00eate sur la salubrit\u00e9 des aliments<\/u><\/strong><\/p>\n<ul>\n<li>L\u2019Agence canadienne d\u2019inspection des aliments (ACIA) a effectu\u00e9 une enqu\u00eate de retra\u00e7age pour d\u00e9terminer l\u2019origine des jeunes pousses d\u2019\u00e9pinards consomm\u00e9es par les trois cas en Ontario et un cas en Colombie\u2011Britannique. Les \u00e9pinards consomm\u00e9s par les cas ontariens ont \u00e9t\u00e9 retrac\u00e9s \u00e0 deux distributeurs. Les deux distributeurs s\u2019approvisionnent en \u00e9pinards dans une serre hydroponique situ\u00e9e dans la province et ne distribuent leurs produits qu\u2019en Ontario. Cette serre approvisionne uniquement ces deux distributeurs avec ce produit. Comme les \u00e9pinards ne sont pas distribu\u00e9s \u00e0 l\u2019ext\u00e9rieur de l\u2019Ontario et que les autres cas ne d\u00e9clarent avoir fait aucun voyage en Ontario, il est peu probable que les \u00e9pinards soient la source de l\u2019\u00e9closion. Le retra\u00e7age des \u00e9pinards consomm\u00e9s par les cas de la Colombie-Britannique a r\u00e9v\u00e9l\u00e9 que l\u2019\u00e9picerie o\u00f9 ils ont achet\u00e9 leurs \u00e9pinards s\u2019approvisionne aupr\u00e8s d\u2019une ferme qui distribue uniquement ses fruits et l\u00e9gumes dans l\u2019Ouest canadien, de sorte qu\u2019aucun autre retra\u00e7age n\u2019a \u00e9t\u00e9 effectu\u00e9. En raison de l\u2019absence de points communs entre les \u00e9pinards consomm\u00e9s, on consid\u00e8re que les \u00e9pinards ne sont pas une source probable de ces maladies.<\/li>\n<li>Le retra\u00e7age effectu\u00e9 par l\u2019ACIA a permis de d\u00e9terminer que les graines de chia des deux marques signal\u00e9es par les cas et identifi\u00e9es dans les relev\u00e9s d\u2019achat des cas, soit la marque \u00ab\u00a0Smile\u00a0\u00bb et la marque \u00ab\u00a0Nature\u2019s Planet\u00a0\u00bb, ont \u00e9t\u00e9 emball\u00e9es dans le m\u00eame \u00e9tablissement et pendant la m\u00eame p\u00e9riode. L\u2019\u00e9tablissement est situ\u00e9 en Ontario. Aucune autre marque et aucun autre produit n\u2019est emball\u00e9 dans cet \u00e9tablissement. Le 5 juin, l\u2019ACIA a commenc\u00e9 l\u2019inspection de l\u2019\u00e9tablissement, qui comprenait la collecte d\u2019\u00e9chantillons de produits et d\u2019\u00e9chantillons environnementaux. L\u2019inspection de l\u2019\u00e9tablissement n\u2019a r\u00e9v\u00e9l\u00e9 aucun \u00e9l\u00e9ment pr\u00e9occupant.<\/li>\n<li>Des \u00e9chantillons ont \u00e9t\u00e9 pr\u00e9lev\u00e9s au domicile de deux cas le 5 juin. L\u2019un des \u00e9chantillons \u00e9tait d\u2019un paquet ferm\u00e9 de graines de chia de la marque \u00ab\u00a0Smile\u00a0\u00bb et l\u2019autre \u00e9tait d\u2019un paquet ouvert de graines de chia de la marque \u00ab\u00a0Nature\u2019s Planet\u00a0\u00bb. Le code de lot du paquet ferm\u00e9 correspondait \u00e0 celui du paquet de graines de chia consomm\u00e9es par le cas. Le 11 juin, les tests r\u00e9alis\u00e9s sur les deux \u00e9chantillons ont r\u00e9v\u00e9l\u00e9 la pr\u00e9sence de <em>Salmonella<\/em>. Le 18 juin, le SGE a r\u00e9v\u00e9l\u00e9 que les \u00e9chantillons \u00e9taient g\u00e9n\u00e9tiquement li\u00e9s \u00e0 la souche de l\u2019\u00e9closion.<\/li>\n<li>L\u2019ACIA a obtenu 40 \u00e9chantillons de l\u2019\u00e9tablissement et du commerce de d\u00e9tail\u00a0: 8 \u00e9chantillons des lots rappel\u00e9s, 20 \u00e9chantillons des lots emball\u00e9s \u00e0 des dates ant\u00e9rieures et ult\u00e9rieures et 12 \u00e9chantillons environnementaux. Quatre \u00e9chantillons des lots rappel\u00e9s (paquets non ouverts) ont \u00e9t\u00e9 d\u00e9clar\u00e9s positifs pour <em>Salmonella<\/em> le 12 juin, et ont \u00e9t\u00e9 trouv\u00e9s g\u00e9n\u00e9tiquement li\u00e9s par SGE \u00e0 la souche de l\u2019\u00e9closion le 19 juin. L\u2019agent pathog\u00e8ne <em>Salmonella<\/em> n\u2019a pas \u00e9t\u00e9 d\u00e9tect\u00e9 dans les \u00e9chantillons pr\u00e9lev\u00e9s dans les lots pr\u00e9c\u00e9dents et subs\u00e9quents de graines de chia, ni dans les \u00e9chantillons environnementaux.<\/li>\n<li>Sant\u00e9 Canada a effectu\u00e9 une \u00e9valuation des risques pour la sant\u00e9 le 6 juin\u00a02020. Un risque pour la sant\u00e9 de niveau\u00a02 a \u00e9t\u00e9 attribu\u00e9 aux deux diff\u00e9rentes marques de graines de chia\u00a0: la marque \u00ab\u00a0Smile\u00a0\u00bb (deux codes de lot) et la marque \u00ab\u00a0Nature\u2019s Planet\u00a0\u00bb (un code de lot), ce qui inclut tous les produits dont la production a eu lieu dans l\u2019\u00e9tablissement les 14 et 15\u00a0avril\u00a02020.<\/li>\n<li>L\u2019ACIA a d\u00e9termin\u00e9 que la mesure appropri\u00e9e de gestion des risques \u00e9tait un rappel de classe I pour les consommateurs et un avis de rappel d\u2019aliments. L\u2019entreprise a accept\u00e9 de proc\u00e9der au rappel volontaire et un avis de rappel d\u2019aliments a \u00e9t\u00e9 \u00e9mis le 6 juin 2020.<\/li>\n<\/ul>\n<p><strong><u>Communications en sant\u00e9 publique<\/u><\/strong><\/p>\n<ul>\n<li>Une alerte de sant\u00e9 publique a \u00e9t\u00e9 publi\u00e9e dans le RCRSP le 25 mai\u00a02020.<\/li>\n<li>Un avis de sant\u00e9 publique a \u00e9t\u00e9 publi\u00e9 sur le site Web du gouvernement du Canada le 6 juin\u00a02020 pour informer le public qu\u2019une enqu\u00eate sur l\u2019\u00e9closion des infections \u00e0 <em>Salmonella<\/em> \u00e9tait en cours et d\u00e9signer les graines de chia des marques \u00ab\u00a0Smile\u00a0\u00bb et \u00ab\u00a0Nature\u2019s Planet\u00a0\u00bb comme \u00e9tant une source des maladies. L\u2019avis de sant\u00e9 publique \u00e9tablissait un lien avec l\u2019avis de rappel de l\u2019ACIA.<\/li>\n<li>L\u2019avis de sant\u00e9 publique a \u00e9t\u00e9 diffus\u00e9 par les cha\u00eenes de m\u00e9dias sociaux de Canadiens en sant\u00e9 et communiqu\u00e9 par les comptes de m\u00e9dias sociaux de l\u2019ASPC.<\/li>\n<li>Un avis de sant\u00e9 publique final a \u00e9t\u00e9 publi\u00e9 le 8 juillet 2020 pour informer le public de la fin de l\u2019\u00e9closion et de la cl\u00f4ture de l\u2019enqu\u00eate.<\/li>\n<\/ul>\n<p><strong><u>Derni\u00e8res \u00e9tapes de l\u2019enqu\u00eate<\/u><\/strong><\/p>\n<ul>\n<li>Selon l\u2019enqu\u00eate \u00e9pid\u00e9miologique, l\u2019enqu\u00eate microbiologique et l\u2019enqu\u00eate sur la salubrit\u00e9 des aliments, l\u2019exposition aux graines de chia des marques \u00ab\u00a0Smile\u00a0\u00bb et \u00ab\u00a0Nature\u2019s Planet\u00a0\u00bb a \u00e9t\u00e9 d\u00e9sign\u00e9e comme une source probable de l\u2019\u00e9closion. La cause fondamentale de la contamination n\u2019a pas \u00e9t\u00e9 d\u00e9termin\u00e9e.<\/li>\n<li>L\u2019\u00e9closion a \u00e9t\u00e9 d\u00e9clar\u00e9e termin\u00e9e et le CCEE a \u00e9t\u00e9 dissous le 7 juin\u00a02020.<\/li>\n<li>Aucun cas suppl\u00e9mentaire comportant une date d\u2019apparition des sympt\u00f4mes post\u00e9rieure au rappel n\u2019a \u00e9t\u00e9
